Active Practice – Transfer from In-Active

REGISTRATION TASKS

To register for Active Practice from the In-Active Practice Register with CPM, you must complete these tasks:

TASK 1: CONTACT CPM
Contact CPM for current Registration Requirements.  You may do this by email, phone or in person.

At this time, CPM will:

  • Direct you to where you can obtain the registration form on the website.
  • Inform you of the cost of your registration fee.  CPM will base your Active Registration fee on the date of your registration (pro-rated after August 1st) and deduct what you paid for the In-Active roster during that registration period.

TASK 2: APPLICATION FORM
Complete the Application for Registration Form (PDF) to apply for registration as an Active Registrant

TASK 3: PROOF OF PRACTICE HOURS

CPM requires that all  physiotherapists have completed 1200 practice hours within the last 5 consecutive years.  The College requires documentation from your previous employer(s) confirming your hours broken down per year.

  • If you work in Private Practice, please ask your employer to confirm your practice hours on company letterhead and address it to CPM
  • If you work in Public Practice, please contact Shared Services and ask for an audit of practice hours.

TASK 4: PROOF OF EMPLOYMENT AND LIABILITY INSURANCE
Submit with your package, a letter from your employer confirming your start date as well as proof of insurance.

  • If you work in Private Practice, please submit a copy of your malpractice insurance certificate with your letter of employment
  • If you work in Public Practice, please have your employer confirm in your letter that you are covered by their liability insurance

 

If you left the Province of Manitoba while on the In-Active Register, you must complete the following two tasks. If you did not leave the province while on the In-Active Register, proceed to task 7.

 

TASK 5: MEMBERSHIP IN GOOD STANDING FORM (IF APPLICABLE – See above)
Submit a Membership in Good Standing Form from the last two College(s) in which you were registered.

TASK 6: CRIMINAL RECORD CHECK (CRC) WITH VULNERABLE SECTOR (IF APPLICABLE – See above)
Submit the original criminal record check document with your registration package.  The CRC must be no older than 6 months from the date of issue. The original CRC will be returned to you.

 

TASK 7: REGISTRATION FEE 
Pay your registration fee (fee amount provided by CPM during TASK 1. CONTACT CPM).

You may pay by:

  • Cash (in person, by appointment)
  • Credit card (by phone or in person, by appointment)
  • Cheque (by mail or in person, by appointment). Please make cheques payable to College of Physiotherapists of Manitoba

TASK 8: SUBMITTING YOUR DOCUMENTS

  • Email documents to info@manitobaphysio.com OR
  • Fax documents to 204-474-2506 OR
  • Mail or hand-deliver (office open by appointment only) all required documents and application fee to:

DECISION

CPM will take up to 5-7 business days to process your application. CPM will send you the following by email:

  • A decision letter that confirms that you have been registered for Active Practice
  • Instructions for printing your Certificate of Registration and Receipt for Registration Fees
